I would use my sata to usb dock and attach it to my my pc and format it, delete any partitions and then put it in the bottom bay and try again.

Possibly the sata port is bad in the backplane card, or the backplane needs to be re-seated?
I would open the server and re-seat the backplane.(be extremely careful not to damage the card)
But if it were me I would connect the drive to the pc again and run chkdsk and or install SMART to test the drive.
There is no need to format the drive just delete the volume's.
So it appears to whs to be a brand new unused drive.
In some cases I have had to run diskpart and clean the drive.
WHS will format the drive.

"Ruben Rocha" said it better then I. I was suggesting a format to see if the drive was good.
Has the system been taken apart? if so The backplane may be damaged.
Goodluck

Not sure if it was ever taken apart.

I have tried reformatting the drive still no difference.

Like I said whs will format the drive again when added to the pool.
It just needs to be a clean drive for whs.
I suggest again to follow my prior post.
